The Primary Health Care Corporation (PHCC) has achieved the highest level of accreditation, the Diamond Level Accreditation Certificate from Accreditation Canada 2024. Entities receiving this accreditation are distinguished by their unwavering commitment to excellence, emphasizing quality, results monitoring, evidence utilization, and implementation of best practices to enhance service delivery.
PHCC has been honored with the first-ever globally recognized People-Centered Care (PCC) Centre of Excellence award from Accreditation Canada. This recognition is awarded to health facilities that prioritize patient and family engagement in healthcare processes and maximize their experiences. These significant milestones are a testament to the corporation’s commitment to providing comprehensive, integrated health care that prioritizes patients’ needs.
PHCC has recently undergone a rigorous assessment by Accreditation Canada surveyors. This thorough assessment examined a range of standards and criteria pertaining to quality, safety, risk management, and ethics in medical and support services, covering areas such as finance, information technology, human resources, infrastructure, and management.
This diamond certificate marks a significant milestone towards success, meeting around 1,100 standard requirements at the management level within the corporation and its 31 health centers.
For her part, Dr. Mariam Abdul Malik, Managing Director of PHCC, said that PHCC successfully achieved the Diamond Level certificate from Accreditation Canada, which is a globally recognized and accredited organization that assists health institutions worldwide in maintaining a consistent assessment of the quality of healthcare and social services offered in accordance with international standards and standards of excellence.
Site Visits to Health Centers
Dr. Amal Al-Ali, Executive Director of Quality and Patient Safety Directorate at PHCC, said that this comprehensive evaluation included site visits to all 31 Health Centers and the Headquarters, where the survey team meticulously reviewed PHCC’s policies, procedures, and practices to ensure standardization and consistency in delivering the highest level of care without compromising patient safety.
Quality and Patient Safety
The assessment focused on the culture of Quality, Patient Safety, and People-Centered Care across all sites. Accreditation Canada surveyors conducted thorough administrative tracers, clinical tracers, patient interviews, leadership and staff interviews, patient and partners discussion groups, and reviews of various evidence including projects, key performance indicators, improvement initiatives, personnel files, and corporate plans. Following this extensive review, PHCC has successfully retained its Diamond level accreditation for an unprecedented third time, achieving an overall compliance rate of 99.8% across all sites.
