Citizens and residents of Qatar can apply for a health card to access services at any of PHCC’s health centers and Hamad Medical Corporation’s healthcare facilities or hospitals.
The card is the same size as a driving license. It contains your ID number, nationality, photo and date of issue and are available to all citizens and residents of Qatar.
Consultations or non-emergency treatments may cost extra but will be subsidized for those who hold a valid health card. Holders of a health card can also have their prescriptions filled at a government-run pharmacy at subsidized rates.
To obtain a health card, visit the nearest PHCC’s health center to your residence. There you need to fill out the appropriate forms. You will be issued a PHCC health file number. Please take with you:
Qatari Adults: A valid Qatar Identification Card (QID), 4cmx3cm photo, and credit/debit card to pay the 50 QR fees.
Qatari Children: A birth certificate (bearing QID number), a valid Qatar Identification Card (QID), a 4cmx3cm photo, Inoculation Card (for 1st time issue), and credit/debit card to pay the 50 QR fees.
GCC National Adult Residents: A Valid Qatar Identification Card (QID), 4cmx3cm photo, the old health card (in case of card renewal), and credit/debit card to pay the 50 QR fees.
GCC National Child Residents: A birth certificate, a valid Qatar Identification Card (QID), a 4cmx3cm photo, Inoculation Card (for 1st time issue for children born in Qatar), the old health card (in case of card renewal), and credit/debit card to pay the 50 QR fees.
Adult Residents: A valid Qatar Identification Card (QID), 4cmx3cm photo, and credit/debit card to pay the 100 QR fees.
Children Residents: A valid Qatar Identification Card (QID), a 4cmx3cm photo, Inoculation Card (for 1st time issue, children born and received their applicable inoculation in Qatar), and credit/debit card to pay the 100 QR fees.
Domestic Staff: A valid Qatar Identification Card (QID), 4cmx3cm photo, A Valid Qatar Identification Card (QID) of the sponsor, a fifty Qatari Riyals fee (As per the rules and regulations of personally sponsored professions prescribed by the Ministry of Interior).
All applicants must submit a copy of a utilities bill (Phone, Water / Electricity) as a proof of place or area of residence.
You must pay using a credit card. No cash is accepted. The health center will either issue your card immediately or ask you to return within a few days to collect it.
Please note, health cards are issued during morning working hours only.
Renewing your health card is a simple process and can be done immediately at all PHCC health centers and HMC hospitals. Please ask our Patient and Visitor Service Center staff to direct you to the right location in each health center/hospital.
Cards can also be renewed online via Hukoomi.
If you lose your health card but it is still valid, Qatari nationals will need to pay QR50 for a replacement and non-Qataris will need to pay QR100 for a replacement.
If you lose your health card and it has expired Qatari nationals will need to pay QR100 for a replacement and non-Qataris will need to pay QR200 for a replacement.
