Scope of Service

This service manages patients diagnosed with cognitive impairment. Cognitive assessment clinics at PHCC Health Centers are conducted by consultant geriatric psychiatrists, consultant geriatricians and occupational therapists from HMC and supported by nurses and reception staff from PHCC. 

Target Group 

All PHCC registered patients who are over 60 and are identified by family physicians with cognitive impairment, patients with mild to moderate dementia. 

Type of Access

Patients diagnosed by PHCC family physicians with cognitive impairment (mild to moderate dementia) will be referred to this service by their physician.
